Our team members are dedicated to providing high-quality, patient centered care where individual care aspirations are respected as the patient him/herself is actually included in the development of their care plan. We encourage family participation to assist the patient in achieving and maintaining an optimal level of health, activity, and independence at home.

Available 24 hours a day, 7 days a week

Discharge planning and facilitation

Care for acute, chronic, and terminally ill patients (non-hospice)

On-going and regular communication with Patient, Physician and Family

SKILLED NURSING “RN & LPN”

Our compassionate team of highly trained nurses provide basic and advanced services including: general health management, medication administration, ostomy care, infusion administration, wound care and a variety of other skilled services.

PHYSICAL, OCCUPATIONAL AND SPEECH THERAPY

SEHC offers Physical, Occupational and Speech Therapy services that deliver high-quality results and produce superior outcomes. From improving mobility, reducing fall risk, muscle strengthening and enhancing verbal communication to reducing the risk of aspiration pneumonia, our Skilled Therapists work with Physicians and Southeast Homecare Clinicians to create a uniquely customized coordinated plan of care to meet each patient’s specific care needs.

MEDICAL SOCIAL WORKERS

Illness, disability and life-debilitating conditions can have an impact beyond a patient’s physical health. Our Medical Social Workers assess the social and emotional factors affecting each patient and their family and caregivers. The assessment is utilized to develop and respond with the appropriate counseling and assistance in finding additional community resources to support the patient’s needs.

HOME HEALTH AIDES

Our Home Health Aides and Certified Nursing Assistants are trained to assist patients with Activities of Daily Living (ADL) such as eating, bathing, grooming and ambulating. Home Health Aides also provide simple emotional and psychological support to the patient and patient’s family to promote a safe and comfortable home environment for the patient The team members work under the supervision of our Skilled Nurses to ensure that each patient's customized care plan is properly fulfilled.
