Integrated Home Infusion Pharmacies provides safe and comprehensive therapeutic infusion services to patients of all ages in the comfort of their home. Our patients benefit by receiving safe and effective therapy in their home vs. traveling to an infusion center. Receiving infusion medication in the home greatly reduces exposure and risk to infections which results in savings by minimizing and/or avoiding costly hospitalizations.

HOW IT WORKS

The Pharmacies intake coordination team verifies prescription orders, insurance eligibility, and benefits to develop an individualized care plan tailored to meet each patient’s unique needs. Throughout the course of therapy, the Pharmacy will ensure comprehensive clinical assessments, monitoring and communication with providers, nurses, and other care professionals to ensure safety and effectiveness of treatment.

How It Works

The IHI Pharmacy intake coordination team verifies prescription orders, insurance eligibility, and benefits, and develops an individualized care plan tailored to meet each patient’s unique needs. Throughout the course of therapy, the IHI Pharmacy ensures comprehensive clinical assessments, monitoring and communication with providers, nurses, and other care professionals to ensure safety and effectiveness. Patients and family members are taught to self-administer whenever possible, allowing the patient to gain independence. The infusion therapy administration time ranges from 30 minutes to 8 hours depending on the drug therapy chosen by the provider.

OUR TRUSTED SOLUTION

01. IV Anti-Infective’s

The Pharmacy provides comprehensive clinical services to treat serious infections in the home for patients of all ages.  Many anti-infective therapies take 30 minutes to 1 hour.  In some instances when multiple doses per day are prescribed, the medication may be administered via an ambulatory infusion pump.  The medication administration method is individualized to the patient taking into consideration the medication properties.

02. Nutrition Support

The Pharmacy provides total parenteral nutrition (TPN) for many medical conditions.  The team  assesses and monitors patient progress toward individualized goals. The pharmacist works closely with the provider to optimize therapy to meet patient nutritional goals, avoid hospital readmission’s and transitioning patient off therapy or to enteral nutrition support.

03. Heart Failure Therapy

The pharmacy provides inotropic therapy to heart failure patients for both palliative care and those awaiting transplant.  Inotropic therapy in palliative care allows end-of-life patients to ease symptoms of advanced disease while being in the comfort of their home.  Our pharmacy and nursing teams work together with the physician to individualize therapy, educate the patient / family on what to expect and transition to an ambulatory infusion pump.

01. Physician Replacement Drugs

The Pharmacy works with patient’s providers to deliver their medication to the doctors office for administration.  Our intake team will verify patient eligibility and benefits for services provided in the physician office.

02. Specialty Infusion and Injectables

The Pharmacy provides specialty medications for chronic inflammatory diseases such as Crohn’s disease, rheumatoid arthritis, ulcerative colitis, plaque psoriasis, and juvenile arthritis.  To minimize and manage Adverse Drug Events (ADEs) the pharmacy follows drug specific protocols.

03. Bleeding Disorder Infusion Therapy

The Pharmacy provides pre-and post-surgery therapy and home therapy for patients of all ages.  Our Pharmacy team works closely with the patient’s physician,  nurse and family in the provision of drug therapy, education, and training.

03. Immunoglobulin

The Pharmacy provides intravenous and subcutaneous immunoglobulin products customized to the needs of the patient. The team works closely with the patient’s health care team to manage adverse drug reactions, infusion rates, and patient health issues.
